For practices running RPM and chronic care programs

Run better care management programs. Without adding more administrative work.

CareDots brings your care programs, patient readings, calls and messages, documentation and billing progress into one place. Your team runs the program; Chief, our AI assistant, takes on the routine calls, notes and follow-ups.

  • CCM, PCM, RPM, BHI and APCM
  • AI scribe for visits and calls
  • Practice phone numbers, calling and texting
  • Epic and Akute connections, CSV import
  • HIPAA ready

Delivery model

Software for your care team. Be clear about who does the work.

Practices ask us whether we run the program for them or give their team the tools. Today CareDots is software your own staff use. Here is what that does and doesn't include.

Which programs can be billed together?

These are the rules NurseDot checks before a month is ready to bill. They follow Medicare (CMS) rules for 2026; commercial and Medicare Advantage plans vary, so check each plan.

  • Together RPM with CCM, PCM or BHI in the same month. The same minutes can't count toward two codes.
  • Not together CCM and PCM by the same practitioner for the same patient in the same month.
  • Not together APCM with CCM, PCM or TCM for the same patient in the same month.
  • Rule RPM device codes: one per 30 days. 99454 needs readings on 16 to 30 days, 99445 on 2 to 15 days.
  • Rule RPM time: 99470 (10 to 19 minutes) or 99457 (20 minutes or more), never both. 99458 follows 99457, at most twice.
  • Rule CCM needs two or more chronic conditions, consent, a care plan, and an initiating visit within 12 months for new patients.
  • Rule Face-to-face visits are billed separately and never count as care time.

Software tracks time and checks rules; it doesn't guarantee payment. Your billing team stays responsible for each claim.

Devices and patients

Readings from home, and the limits, up front.

What works today

  • Bluetooth meters through PatientDot. Standard blood pressure cuffs, glucose meters, scales, pulse oximeters and heart rate monitors pair in the patient app.
  • Typed readings. Patients without a meter that pairs can enter readings by hand.
  • Readings land in the chart. Your team sees them in each patient's Vitals view, and Chief's Watchlist flags patients who need attention.
  • No app store needed. PatientDot installs from the browser on a phone, tablet or computer.

What to know first

  • Pairing needs Chrome on Android or a computer. Bluetooth pairing doesn't work on iPhone or Safari; those patients type readings in.
  • No cellular devices yet. Meters that send readings over their own SIM aren't supported today.
  • We don't supply devices. Ordering, shipping, connectivity and replacements stay with your practice or your device vendor.
  • Ask about FDA clearance per device. It depends on the meter you choose, not on CareDots.

AI and workflow

What goes in, what Chief does, what you get.

Visit and call documentation

In
A visit or phone call recorded with the call scribe
Chief
Writes the transcript, a draft SOAP note, tasks and a message for the patient, and scores call quality
You get
A note to review and sign instead of one to type

Care plans

In
The patient's chart, visits and goals
Chief
Drafts a care plan from your templates
You get
A plan your clinician edits and assigns to the patient

Finding eligible patients

In
Diagnoses in the chart
Chief
Checks each patient against each program's rules
You get
A list of patients who may qualify

Consent and first visit

In
An eligible patient and your approved call script
Chief
Calls inside calling hours, explains the program and its cost, records consent if you allow it, and offers times for the first visit
You get
An enrolled patient with a booked visit, or a task for your team

Incoming calls

In
A call to the number you put on the AI
Chief
Answers with the name you give it, books appointments and hands the caller to a person when asked
You get
Fewer missed calls, with a summary of each one

Monthly care time

In
Your team's time with each patient
Chief
The care timer counts it toward each CPT code and flags what's missing
You get
A month that is ready to bill, or a clear list of what isn't

Calls and the care team

One phone system, kept apart from your triage line.

Practice numbers

Get practice numbers in NurseDot. Give a team member their own line that rings in the app and can forward to their phone.

Your number on outgoing calls

Chief's outreach calls come from your practice number, and each call says its callback number. No personal cell numbers.

Recordings and summaries

Scribed calls keep a transcript and a summary on the patient's timeline, with call time logged.

Calls and texts in one timeline

Two-way calling and texting, with patient app messages, in one record per patient.

Calling hours and limits

Chief calls only 9 am to 7 pm in the patient's time zone, at most once a day, and hands over to a person for anything clinical.

Team visibility

The dashboard shows calls, tasks, care time and call quality, with a per-person view for admins.

EHR and connected workflows

Integrations, by what's live today.

Does pricing change with volume?

We don't publish volume tiers. Ask for an itemized quote for your team size and patient numbers.

What's charged separately?

Devices and their connectivity aren't sold by CareDots. Phone numbers and call usage, onboarding and support terms are listed in your itemized quote.

Long-term contract or month-to-month?

Contract terms are set in your agreement. Ask on your demo call and we'll put them in writing.

Practice economics, illustrated

An illustration, not a forecast and not guaranteed revenue. It uses 2026 Medicare national allowed amounts (the total of Medicare's 80% and the patient's 20% coinsurance), assumes every patient meets each code's requirements every month, and leaves out locality, denials and payer mix.

Illustrative Medicare allowed amount—
CareDots cost—
Your staff cost—
Difference, before other costs—
Assumptions used
  • CCM: 99490, $66.13 a month
  • RPM: 99454 ($52.11) plus 99457 ($51.77), $103.88 a month
  • PCM: 99426, $67.80 a month
  • BHI: 99484, $57.45 a month
  • National non-facility amounts from the CY2026 Medicare Physician Fee Schedule (conversion factor $33.4009), before any locality adjustment.
  • CareDots: $99 per user (at least 5) plus $3 per patient per program.

Getting started

How a practice starts, step by step.

  1. AssessWe look at your practice, programs and current tools with you.
  2. ConfirmAgree which programs, integrations and devices you'll use, and who on your team does what.
  3. Set upConnect your EHR or import patients by CSV, set up phone numbers, and approve Chief's call script.
  4. TrainWalk your team through NurseDot and DoctorDot, and prepare how you'll invite patients to PatientDot.
  5. LaunchStart with a first group of patients, watch adoption on the dashboard and adjust.

Switching from another vendor? Patients can come across by CSV, and your existing devices can keep working if they pair over standard Bluetooth. Timelines depend on your EHR and team. We'll give you a realistic one on the demo call, not a promise here.

Questions practices ask

FAQ

Do you run the care program for us?

No. CareDots is software your team uses, with Chief handling routine calls, drafts and follow-ups. We don't provide nurses or care managers today.

Do patients need a smartphone or Wi-Fi?

To send readings automatically, yes: PatientDot pairs Bluetooth meters on Android (Chrome) or a computer. Patients can also type readings in. Cellular devices that work without a phone aren't supported yet.

What if a patient stops sending readings?

Billing shows each patient's progress toward each code, so a month without enough reading days isn't marked ready for RPM device codes. Ask us how such patients are counted in your quote.

Which EHRs do you work with?

Epic and Akute today, plus CSV import from any system. athenahealth, Elation and Oracle Health (Cerner) are planned, not available yet.

Can we bill RPM and CCM for the same patient?

Yes, Medicare allows both in the same month, as long as the same minutes aren't counted twice. CCM and PCM can't be billed by the same practitioner for the same patient in the same month. Check commercial plans individually.

Does the software guarantee we get paid?

No. It tracks care time, checks billing rules and flags gaps. Payment depends on your documentation, the patient's coverage and the payer.

What caller ID do patients see?

Your practice number. Team members can have their own direct line, so patients don't call personal phones or your triage line.

How is staff trained?

We walk your team through the apps during setup. Ask us what training is included in your quote.

Can we talk to a current client?

We don't publish references on this site. Ask on your demo call.

Is it HIPAA compliant?

CareDots is HIPAA ready. Ask us about a business associate agreement and how your data is handled.

Let's map out the right care management model for your practice.

A short call: your programs, your team, your EHR. We'll tell you what fits and what doesn't.
